  • Transcript Request Form (PDF)

Transcripts (official and unofficial) are released ONLY upon the signed consent of the student. Upon receiving a signed request, we may mail, fax, or email the transcript, as directed in the request. The student should verify with the recipient of the transcript if the transcript needs to be official or unofficial (see description below), and the acceptable mode of delivery.

The request should include:

  • the student’s full name as when she/he attended
  • approximate dates of attendance at Nebraska Wesleyan
  • student ID number or social security number
  • birth date
  • name and address (or fax number or email address) of where transcript is to be sent
  • student’s signature (required)

This request may be mailed, faxed or scanned/emailed to:

Registrar’s Office
Nebraska Wesleyan University
5000 Saint Paul Avenue
Lincoln, NE 68504-2796
Fax: 402-465-2565

Or email a scanned signed request to transcripts@nebrwesleyan.edu.

Transcripts are normally processed within two to three working days.

Official Transcripts

An official transcript bears the seal of the university. When a transcript is released to a student it will indicate it is “Issued to Student”. (Verify with the recipient if this is acceptable.) Because the university seal is not transmitted electronically, faxed or emailed transcripts should not be considered official. A student’s account with the business office, library and other pertinent campus offices must be paid in full before being entitled to an official transcript.

The first official transcript for each student is free of charge; after that there is a fee of $5 per copy. If six or more transcripts are ordered at one time the fee is reduced to $1 for the sixth and following transcripts.

Unofficial Transcripts

There is no charge for an unofficial transcript, which may be mailed, faxed, or emailed.
